What is a Neurologist?
A neurologist is a medical doctor who has completed medical school, one year of internship and at least three years of training in the treatment of disease of the brain, spinal cord and nerves. 
 
Do I need a referral to make an appointment? 
No referral is needed. However, please have any medical records (office notes, lab results, MRI reports etc.) that are pertinent to your current problem faxed to our office prior to your visit. 
 
What do I need to bring to my first appointment?
1. Driver’s license and/or another form of picture ID
2. Insurance Cards (if you are not insured, payment is required prior to any services rendered)
3. Updated list of medications 
Please Arrive 20 minutes ahead of time to fill out necessary paperwork. 
 
Will I be charged to reschedule? 
There is no rescheduling fee. 
 
 
 
 
How do I cancel my appointment? 
ALL appointments must be canceled at least 48 hours prior to your visit or you will be subject to a cancellation fee of $60 (excluding emergency hospitalizations). Please call our office if you encounter any scheduling conflicts that may affect your appointment time. If it is after hours, feel free to leave a message with our answering service. 
 
Does the doctor fill out disability and/or insurance claim forms? 
Yes. However, Neurological Associates charges a $50 fee for all handicap parking, disability papers and insurance claim forms prior to receiving the requested information. 
 
How do I get my Rx refilled?
Please call your pharmacy and request them to fax a prescription refill request form to (352) 787-7216. 
 
What forms of payment are accepted?
